This is the first step in preparing a document for Design Online: adding variable data blocks to an existing PDF file. This is done in Adobe Acrobat Pro (versions VIII through DC) using the free PDFlib Blocks plugin, available for both OSX and Windows.

Acrobat Pro on Windows exists in 32-bit and 64-bit versions. You must download the version of the PDFlib plugin that matches your version of Acrobat Pro.
The image below shows how to check whether your Acrobat Pro is 32-bit or 64-bit using the Windows Task Manager. The Task Manager will clearly display "32-bit" if your version is 32-bit:
